Jacqueline supports clients through emotional, social, and academic/workplace concerns. Specifically, she helps clients navigate anxiety, panic, depression, new mental health or medical diagnoses, identity, grief/loss, family change, life transitions, parenting, behaviors/dysregulation, attention/focus concerns, and skill building.

WELLNESS PROCESS

  • To get started, the first step is to submit a request using the contact us tab or you can reach out directly via email, call, or text. We know getting started in therapy can feel scary and overwhelming. We want to make things as simple as possible for you. Please provide your therapist with your preferred name to set up a Simple Practice portal. To set up your client portal, your therapist will need a phone number and email address. Simple Practice will send you an email welcoming you to set up an account. If you don’t see an email from Simple Practice, please check your spam folder. Once you create your account, you will notice some consent forms and documents you will review to get started. Documents for review include:

    After completing consent forms, you will complete a new client questionnaire to provide background information to help guide the therapist in your first appointment, which is an intake session. You will also complete some symptom checklists to help your therapist determine next steps in developing treatment goals and plans specific to your needs.

  • You have taken a very positive step by deciding to seek therapy. The outcome of your treatment depends largely on your willingness to engage in the process, which may, at times, result in considerable discomfort. Remembering unpleasant events and becoming aware of feelings attached to those events can bring on strong feelings of anger, depression, anxiety, and other negative responses. Our wellness is an ongoing process that takes time and intentionality to feel better. I cannot promise that your behavior or circumstance will change. I can promise to support you and do my very best to understand you, identify any repeating patterns in your life, and to help you clarify what it is that you want for yourself and your overall wellness.

  • Scheduled appointments can be viewed in Simple Practice. To schedule an appointment please call, text, or email provider to find openings. To cancel or reschedule, please reach out directly to provider. Please note that appointments canceled with less than 24 hours notice will be subject to late cancellation fee.

  • Clients create an account in Simple Practice, our client portal, where you will upload your insurance information and credit card information for insurance coverage and any copays or deductible payment required under your specific insurance plan. If you plan to submit for reimbursement for out-of-network provider, please contact your insurance company to confirm what is required to access benefits. Private pay options are also available.

  • Legal parents/guardians will complete consent paperwork in Simple Practice to help set up their minor with services. Parents will be responsible for scheduling, billing, and transportation to help support their child in receiving treatment. If you have questions about informed consent or any of the privacy agreement documents as you begin therapy, please ask us! It is so important for young people to have a safe outlet to explore their feelings and concerns. We want to work in partnership with parents to create safe environments to support children and families in accessing resources, connection, and treatment.

  • Please note that our provider and scheduling line is not set up to be used in the event of crisis. If a true emergency situation arises, please call 911, 988, or immediately seek services with your local emergency room. You may also wish to connect directly with the Crisis Response Unit in your County. Crisis Response Units can respond after-hours for child protection screening and response as well as after-hours emergency social services.
